Custom fileds on form and link header field

If you see website you can see that I modify something

Well my first question is about filed on the wensite "signo del zodiaco" i want to show a customize dropdown field with a list of text

Second question is about when comments are show, It show a list of headers fields, if you see I modify order, but I need to show at the end "signo del zodiaco" field.

Third question is about how it show comments. Well I'd like to show a link on an any header fileds on comment and later when click It show in another page with this structure www.tarotsiono.es/consultorio/consultorio-1.html

It's possible to do this
