double bbcode buttons

Hi Guys,
I've got a strange problem.
I have installed Jcomments, and for some reason the bbcode buttons display twice in two identical rows one above the other.
In addition the box with the the character counter at the bottom of the comments also displays twice in two identical rows, one above the other. If I switch the character counter off, the boxes still display twice but empty. (image attached)
How do I make it display only once as it should?
Any help will be highly appreciated.

and one short question. I would like to change the texts "Refresh Comments List" and "Comments" at the very top, but I dont know in which file they are.

It looks like you've tried to insert comments form twice at the same page... Could you show me your site? What JComments version do you use?

Hi Smart, the installation was on localhost. I wanted to check that it works well before installing on live site.
I will install on live site and will leave a notification message.

Hi Smart,

I have installed the Jcomments in my live site:
www.enlightenment.co.il

The site is in RTL and style.css and tpl_form.php have been slightly modified but the displaying of the double bbcode buttons and character counter boxes happened before any modifications were made.

If you can figure out whats going on please let me know.
